Informed consent to the processing of personal data

As a legal representative, I give my consent to the Czech-English children's group Shining smile, z.s., registered office at U Kapličky 44, Dobřejovice 251 01, to collect, process and record the below personal and sensitive data about my child in the sense of the European regulation GDPR regarding the processing of personal data.

I give my consent for the entire period of my child's attendance. I have been informed about my rights, in particular about my right to revoke this consent at any time, even without giving reasons, about the right of access to this data, the right to correct it, about the fact that the provision of personal data is voluntary, about the right to be informed about it , which personal data were processed and for what purpose. I further confirm that I have been informed that if I find out or believe that the children's group is processing personal data that is contrary to the protection of the private and personal life of the data subject or contrary to the law, especially if the personal data inaccurate with regard to the purpose of their processing, I can ask the mentioned institutions for an explanation, demand that the children's group remove the state thus created.
